1.      PGY-1 residents will always have a “direct supervision or indirect supervision immediately available”. Home calls are considered “indirect supervision available” and will not satisfy ACGME requirements for PGY 1 supervision. Supervising physician can be either a faculty member or more senior resident.

2. Programs must design clinical assignments to minimize the number of transitions in patient care. They refer to studies detailing that many errors occur during patient sign-outs.

3. Adopt fatigue mitigation processes. No longer are lectures solely appropriate, but ACGME wants to see action (i.e. scheduled naps between 10 p.m. – 6 a.m., back-up call schedules).

4. PGY-1 residents are not permitted to moonlight.

5. Consecutive hours:
  •PGY-1 residents – Maximum 16 hours (+0 hours transition)
  •PGY-2 and up – Maximum 24 hours (+4 hours transition)

6. Minimum of one day free of duty every week, averaged over four weeks.

7. PGY-1 residents should have 10 hours and must have 8 hours free of duty between scheduled duty periods. Intermediate-level residents should have 10 hours and must have 8 hours free of duty between scheduled duty periods. Senior level residents should have 8 hours between scheduled duty periods.

8. Maximum of 6 consecutive nights on night float.

9. PGY-2 residents and above must be scheduled for in-house call no more than every third night, no averaging.
