The 2 words are used interchangeably, FMGs specifically are those who hold non US citizenship.
As the reader can see from the above diagram, the structure of GME is fairly simple. It is important to note that at the end of residency doctors can work at an independent level as attending i.e. consultant equivalent in other countries. Not all doctors choose to go into fellowships and subspecialty training. In fact, there is a thriving market for Internal Medicine attendings who are hospital based (or Hospitalists). Moreover, US training gives a sense of security which you do not find in UK or Ireland. Residency and Fellowship programs all have a specific exit point
